37 hours per week, £19,875-£21,798 per annum, working for Southdown, This is an exciting chance to be part of an innovative project based in GP Practices in Hastings. GP’s have identified that there are a significant number of repeat visits from vulnerable local residents who state that they are experiencing mental distress due to issues with their housing, finances, relationships, unemployment and general wellbeing.

The GPs are finding it difficult to offer medical solutions to what appears to be the impact of social and economic issues.  The aim of the service is to offer alternative support options to improve patient health and wellbeing and subsequently reduce the number of GP appointments that individuals require.

Working closely with the Coordinator and team, this post will provide direct support to clients referred to the service. You will need experience of providing frontline services and be skilled at working in a solution focused way to enable clients to successfully address issues that they feel are negatively impacting on their health.

Ideally you will need to have an extensive existing knowledge of services and other support providers in the Hastings area.
